No Insurance...No Problem

Good oral hygiene is important, and we don’t want your teeth and gums to suffer because you don’t have dental insurance.  We have designed a in-house dental advantage plan to give individuals and families without dental insurance an affordable solution. You can even sign up if you have a dental emergency and require unplanned dental work.

The Cheboygan Dental Group Dental Care Plan provides affordable care without the limitations of traditional dental insurance, helping patients save money.

Plan Duration

Plan duration is for 1 year from enrollment date and will renew yearly, unless a written cancellation notification is received prior to renewal date or you fail to pay the yearly fee. The plan may be discontinued by Cheboygan Dental Group at any time upon written notice to you, at the end of any 1-year term.

Plan Payment Options

One-time yearly payments can be made with cash, check or credit card. All patient payment portions for additional dental services, not included in the preventive care services outlined here, are due at time of service in order to receive the 10%, 12% or 15% discount provided by the plan.

Payment plans for additional dental services are available on request with approved credit through Care Credit.

Plan Participation Guidelines

Cheboygan Dental Group Dental Care Plan is not dental insurance, but a dental care membership plan and is not transferable to other dental offices or to any other person. The Cheboygan Dental Group Dental Care Plan is only available to patients who do not have existing dental insurance, out of network or choose not to utilize their insurance. It is the patient’s responsibility to schedule and keep their appointments. NO REFUNDS WILL BE PROVIDED FOR DUES PAID, including failure to schedule and maintain appointments. Please notify our office at least 24 hours in advance if you must change a scheduled appointment. Fees for dental services other than the preventive care services included in the yearly fee, may change at any time.

For patients with regular cleanings and absence of periodontal (gum) disease.

       $315PER YEAR ($208 savings)

Includes:

§  Dental Cleanings (2 per year)

§  Doctor Exams (2 per year)

§  All needed xrays

§  Fluoride Treatment  (2 per year)

§  Emergency Exams (2 per year)

Plus:
Year 1:  10% off all additional dental services.

Year 2:  12% off all additional dental services.

Year 3:  15% off all additional dental services.
